By submitting this form I am giving my permission for my child to participate in the Reformed Church of Bronxille Youth Ministry sponsored events for the Program Year 2025-2026.  
I understand that participating in these events may involve certain risks.  On behalf of myself and my child, I hearby release, and forever discharge and agree to hold harmless The Reformed Church of Bronxville, the staff, agents and volunteers, from any and all liability, claims or demands of any nature whatsoever which may arise in connection with my child's participation in, or being transported to or from such events.  I give my permission for my child to ride in the Church Van to offsite Youth Ministry programs.  I understand that the Reformed Church is no longer responsible for the supervision of my child, whether or not my child leaves the church grounds at the conclusion of the event.  I understand it is my responsibilty to arrange for my child's transportation to and from Youth Events whether by car or walking.
